Residency Affidavit & Change of Address Forms

If you are attending Vance using a Residency Affidavit, it must be renewed and notarized every year before the start of school. Go to this link for the form and documentation needed: RESIDENCY AFFIDAVIT

If you moved, you must change your address with the school. Go to this link for the form and documentation needed: CHANGE OF ADDRESS

You can submit the information to us electronically by fax (980) 343-5286 or by email to rhyshellal.stitt@cms.k12.nc.us

If you wish to come to the school in person to file the paperwork, you must wear a mask. Please call the school to setup an appointment. The office is open to the public 2 hours each day Monday to Wednesday as follows.
