Athletic Forms Due Prior to Participation in Tryouts!
All Students must have their athletics packet complete and submitted prior to their first day of tryouts. This year, athletic forms have gone digital! Sign up for a Dragonfly account to get access to this year's athletics forms. Follow the directions below.
Forms due include:
- CMS Pre-Participation Form
- Student and Parent Honor Code Form
- Physical Evaluation
- Release Form
Physicals are available with no appointment needed!

For your convenience, Novant Health offers sports physicals. You can
walk in anytime – no appointment is needed. Or, you can call and
schedule a visit at a time that works for you.
Cost:$45 (no insurance accepted)
What to bring: Any necessary forms.
Participation Fee
Participation Fee of $75 is due once students are rostered for a sport and must be paid prior to the first game. Completion and approval for
free and reduce lunch form (Linked)may result in this fee being waived. Once the fee is paid, there are no refunds. All fees support funding for middle school sports. Pay fees via the button at the top of this website:
