​Elizabeth Lane Elementary has a full-time nurse, Amy Lattimore, BSN, RN. She can be reached by calling the school at 980-343-5700 or via email at amy.Lattimore@mecknc.gov. 

  • If your child should need medication while at school, a medication authorization form needs to be completed by you and your doctor. This includes prescriptions, over the counter medications and other items such as cough drops, and lotions. Links to these forms are listed below. Please note there is a generic formas well as forms specific to chronic illnesses. 
  • If your child needs a medication during the day and you would like to deliver it to the school, you must be the one to administer the medication. This is a CMS/Mecklenburg County Policy.  

 

Requirements for Kindergarteners and students new to CMS  

Each child new to CMS will need a Health Assessment (physical) dated within the 12 months prior to the first day of school. A copy of your child's up to date immunization record is also required. (Copies of immunizations printed by former schools will not be accepted. They must be documents from a medical provider, etc., or copies of those will suffice.) These forms need to be submitted within 30 calendar days of the start of school.

AND ONE of the following if medication is needed for seizures while at school:

Reporting Covid Positive Cases and Exposures

Our school Covid team will communicate individually to provide return dates and criteria for individuals who have tested positive or are quarantining due to a close contact exposure.  If your child has tested positive within the past ten days, or has been in close contact with an individual who has tested positive within the last 14 days, please report this information as soon as possible by emailing our team:

Using the team email will ensure that all members of our Covid support team have the information and can work together to provide you with timely information.  Our team will also alert your child's teachers so that assignments can be provided during the time away from school.
