Change of Address, Transfers & Withdrawals

Change of Address: If you are moving within the Elon Park school zone, please see the CMS Change of Address site for instructions. Return the completed Change of Address Form and proof of residency documents to the Elon Park front office.

Withdrawal/Transfer: If you are moving or attending a private or charter school, please visit Elon Park Elementary School front office to complete a withdrawal/transfer form. Note that your new school will reach out to Elon Park for student records and we will respond as quickly as possible. We will not withdraw students until Withdrawal Form has been completed and we have received request for records from new school.
