Documents needed to Enroll
New Student Enrollment Form (for the current school year and future years)
English ~~ Spanish
Certified Birth Certificate (no hospital, souvenir or Mother copies) Safe Schools Enrollment Declaration - English | Spanish ( Required for students transferring into CMS) Current Immunization records (Complete record required by 1st day of school entry) Health Assessment (Required for new pre-k and kindergarten students by 1st day of school entry)
Forms for the current year (2019-2020) must be submitted to the student's home school. Forms for the upcoming school year (2020-2021) can be submitted to any CMS school, any CMS Learning Community Office, or the Student Placement Office (until May 15).
Home School Students
If student was home-schooled, a copy of the home school license and the following assessments/ documents will need to be submitted prior to being placed into a classroom:

Current information about the reading and math programs used while home-schooled, including textbooks used and work samples from the student(s)

Summative/End of grade standardized test data from the end of the year when home-schooled. The parent should provide test scores from a nationally standardized test or equivalent measure that are adequate to determine proper grade placement

If the student does not have standardized or end of year test data, the student will have to take the following assessments prior to being placed into a classroom at Highland Creek: Reading 3-D assessment if K-2nd; Reading A-Z if 3rd-5th; a Reading STAR test; a Math assessment with the end of year standards for the previous year of instruction.
Age Requirements
In compliance with North Carolina law students must be 5 years of age on or before August 31st to be considered for Kindergarten. Students applying for the Montessori schools Pre-K program must be 4 years of age on or before August 31st in order to be considered.
Immunization and Health Assessment Requirements
In compliance with North Carolina law, all students entering kindergarten must present a health assessment (physical exam report) by the first day of school entry. The assessment must have been completed within 12 months of school entry. In addition, North Carolina law requires parents/guardian to present certification of immunizations on the first day of school entry. If documentation is not presented, parents and/or guardians have 30 calendar days to provide documentation or the student shall be excluded from school until proof is provided.
International Students
The mission of the Charlotte-Mecklenburg Schools’ (CMS) International Center is to address the needs of culturally or linguistically diverse students, providing them with equal access to effective participation in the educational programs offered by the Charlotte-Mecklenburg Schools.
Address Changes
**CMS REQUIRES 3 PROOFS OF MECKLENBURG COUNTY RESIDENCY WHEN YOU ARE CHANGING YOUR ADDRESS
You can use the ENROLLING YOUR STUDENT link HERE to access the CMS required proofs of residence. We CAN NOT change your address unless you have all 3 proofs of residence at the time you are requesting an address change.
All address changes must be completed in person.
This policy may affect transportation to and from school for your child. Please be advised that you may have to provide transportation for your child temporarily.
CMS Handbooks, Forms, and Notices
Again this year, all CMS required forms and the Parent-Student Handbook will only be available online. The move to online forms will save the district printing, processing and shipping costs. It also aligns with CMS' efforts to conserve resources and limit paper use.
