Transportation Change Form

This form is for requesting a change to your child's transportation because of an emergency situation only. Transportation changes must be made prior to 12:00 pm. Requests made after 12:00 pm will not be accommodated.

Parents:
Upon completion of this form, your child's teacher will be contacted regarding a change in transportation. After the change is accepted and confirmed, the office staff will write a confirmation email back to the parent. If you do not receive a confirmation by 3:00 pm, please follow up with the school by calling (980)343-3625.

HFES Transportation Protocol:
Transportation changes MUST be written in the student agenda or as a note with advance notice. Students without a written request will sent home on their normal mode of transportation. Changes are not accepted via phone, email, nor classroom communication system.
