All students attending school must be up-to-date with their immunizations and must have proof of required immunizations in order to attend school. All students new to NC public schools are also required to present proof of a Health Assessment (see document above).
Without proof, students will not be able to attend school after 30 days, of first beginning, and will not be permitted to return until immunizations and/or the Health Assessment is up-to-date and on file at the school. There are several ways to get immunizations and the Health Assessment for your child...
Health Assessments must be completed by a health care provider. Your child can receive immunizations from their regular doctor/health care provider or at the Health Department during regular office hours, Monday through Friday. The Health Department is open until 7:00 pm on Wednesdays. To make an appointment at the Health Department, please call (704) 336-6500.
Be sure to take your child's immunization record, insurance or Medicaid card, and at least one form of identification with you to your appointment (fees may be charged).
