To ensure your child has the health support they need, our district requires that specific forms be completed and brought to the school nurse to ensure your child receives proper health care and support.


 

7th Grade Immunization Information

Tdap Vaccine and Meningococcal Conjugate Vaccine for all 7th-grade students.

*North Carolina state law requires all 7th-grade students to receive one dose of Tdap (tetanus, diphtheria, pertussis) and one dose of meningococcal conjugate vaccine. Your child will need to bring a copy of the shot record to school on the first day of 7th grade.  After the 30th day of school, students who are not in compliance will not be allowed to attend school, per state law.  Please contact your health care provider or the Mecklenburg County Health Department to schedule an appointment.
