CMS requires that you now submit 3 proofs of Mecklenburg County residency when you are changing your address.

·         Complete the Change of Address form information.

·         Address changes cannot be made unless 3 proofs of residency are provided at time of request.

·         All address changes must be completed in person.

·         Please be advised that an address change may affect transportation.
