Welcome to the beginning of the new school year!

This is a very important reminder about your child. North Carolina law requires
that parents provide the school with a certified record of your child’s immunizations
and current physical exam (health assessment) on or before the first day of school.

If your child has not had the required shots and/or physical exam, or if you are not
sure, please contact your doctor/health care provider as soon as possible. You
may also contact the Mecklenburg Health Department for an immunization
appointment at 704-336-6500. More information and community locations for
immunizations and physical exams is available on the CMS website at
www.cms.k12.nc.us or call the school at 980-343-6088. Thank you for your
