Families enrolling for the 22-23 school year must complete and print enrollment form and bring requried documentation to the Elon Park Front Office between 10AM-2PM Monday-Thursday.  Please see this site for enrollment form:  https://bit.ly/CMSPlacement

Please follow these steps to start the enrollment process:

1. Complete the New Student Enrollment Form:

2.  Complete the Health Assessment and Provide Immunization Records as well as complete the Notice of Requirements Form:

In order for children to enter school in North Carolina, they must have a health assessment signed by your child's physician and up-to-date immunization records.

3. Collect Required Documentation (see page 2 of Enrollment Form)
